Conduit Health's $17M Series A: Building the “Easy Button” for Home Healthcare

We started Conduit Health with a simple goal: make it easier for people to get the medical supplies and services they need to live well at home.
Before Rocky and I were founders, we were friends: two teenagers on a gap year between high school and college who couldn't have guessed we'd one day build a company together. A decade later, after years spent inside health tech and startups, we reconnected over the opportunity to reshape post-acute care. Rocky was working as a PM in health tech, where he saw patients sent home from the hospital without the supplies they needed to heal, simply because getting them was too complicated. I was thinking about a tangential health-related idea, and knew Rocky was exactly the person to call.
What problems are we actually solving?
For 50 million Americans, getting insurance to cover home medical supplies is a nightmare. It’s centered around the concept of a primary care doctor willing to do paperwork on behalf of their patient at home, for no compensation, but this theoretical PCP to patient relationship doesn’t actually exist anywhere in our country. Paperwork is supposed to get faxed (yes, faxed) to an insurer for prior authorization, adding weeks to the process. Finally, a medical supply company fulfills the order, if they can, and another 7 business days later, the supplies arrive. To recap: five or more parties, none of whom are connected, and over 50 days of waiting. Crazy but true statistic: less than 10% of patients eligible for covered supplies ever receive them. The system isn't just broken, it's a disaster that appears not to factor in patients at all.
So we built something new. We like to say Conduit Health is the "Easy Button" for getting things covered in the home. We've created a front door that’s universally accessible, fitting snugly into existing workflows. Partner organizations and their patients can email us, or visit our website, or use our portal, or call us, or even ask us to send an AI-agent into their system to retrieve information every hour. They will occasionally even fax us, and that’s okay: our front door was painstakingly designed to meet people where they are, because that is what the market needs.
Behind the scenes, CareOS - our agentic intelligence layer - uses AI to automate the complex, insurance-specific workflows that have paralyzed this industry for decades. We vertically integrate telehealth, insurance navigation, and fulfillment. Complex business logic builds on itself, so with every case that flows through the system CareOS gets even better. CareOS enables us to offer hundreds of items and services at no cost, handling the prescriptions, paperwork, and insurance approvals, and deliver everything right to the patient’s door.
Since launching, we've delivered supplies to more than 50,000 patients across 100+ health plans and nearly 90 million covered lives. We saw more patients in February 2026 alone than in our entire first half of 2025. Every day, our team helps more people receive the care and supplies they need faster and without friction — and the work is worth it, especially when we hear stories like this:

Stories like Jackie's serve as a constant reminder of what our work is really about. Our mission is to make the experience of getting insurance-covered supplies as easy as getting a prescription filled — and in doing that, we're restoring independence and adding dignity in the moments that matter most.
